For the proclamation template, fill in your city/ mayor's name at the bottom. Then copy and paste into your mayor's online submission form for proclamation requests. For anyone wanting to submit a governor's request, email email@example.com to see if your state has a representative yet.

2. Tell why you want the library to participate (share short personal story)
3. Leave your contact information
